Get setup as a personal representative. If your child is 17 or younger, or is mentally incapable of making their own decisions, please contact Select Health Member Services to begin the process. Privacy is important to us. We will need to verify that you are who you say you are before we can give you information. This is to ensure the protection of the child's information and is in accordance with HIPAA guidelines.
