How does Care Management work?

First, you will be paired with a dedicated Care Manager from our team of experienced healthcare professionals—including nurses, social workers, and care navigators. Medicaid members also have access to community health workers.

From there, we’ll get to know you and your health needs, and recommend care options that fit your situation. We’ll work around your schedule and can meet with you on the phone, through video chat, or by email.

What are the benefits of Care Management?

Care Management can lead to better health and reduced costs through regular preventive care and support with chronic conditions like asthma, diabetes, or heart disease. Care Managers are advocates for you and your health—a trustworthy source of information.
